Getting Started

Your first session at Performance-Focused Physical Therapy is a full deep dive into your story, how you move, and what’s driving your symptoms or limitations. By the end, you’ll leave with a clear understanding of what’s going on and a personalized plan moving forward.

Currently, yes.
At this time, all patients must have a prescription for physical therapy from a licensed provider (MD, DO, NP, or PA) before starting care. Although New York allows direct access to physical therapy, providers must first meet a 3-year practice requirement before treating without a referral. If you need help obtaining a prescription, we’re happy to help guide you.

Scheduling an appointment is done directly through the booking links on our website.
If you’re using out-of-network insurance benefits, you’ll first complete a quick intake form so we can collect your insurance information and run an eligibility check. This allows us to review your benefits with you before confirming your first visit.
If you’re cash-pay, you can book your appointment right away.
All scheduling and intake is completed through Spry, our secure patient portal and EMR system.

Frequency depends on your goals and condition, but most patients at PFPT are seen 1–2 times per week to maximize results and ensure consistent progress.
Insurance and Payment

At PFPT, we choose to stay out-of-network because insurance reimbursement rates don’t support the one-on-one, high-quality care model we believe in. Being out-of-network allows us to spend full, undivided time with each patient and avoid the limitations that often come with insurance-based volume care.

Physical Therapy at PFPT is $225 per 60-minute session, whether it’s your initial evaluation or a follow-up visit. Each session is fully individualized, combining hands-on care, movement assessment, exercise, and education to address the root cause of your issue and help you move and feel better long term.

PFPT operates as an out-of-network provider, meaning you pay at the time of service and your insurance may reimburse you directly depending on your specific plan. Being out-of-network does not necessarily mean “not covered.” We help simplify the process by verifying your benefits and reviewing your potential out-of-network coverage and reimbursement information upfront, so you understand your expected benefits and costs before confirming your appointment.

Yes—both HSA and FSA funds can be used for physical therapy at PFPT. A Health Savings Account (HSA) lets you use pre-tax dollars you’ve saved in a personal account for qualified medical expenses, while a Flexible Spending Account (FSA) is a similar pre-tax benefit account typically offered through your employer for medical costs within a plan year. In both cases, reimbursement is based on your invoice or superbill as proof of care provided and payment, not insurance approval or coding review.

Yes.
After each visit, you’ll receive a superbill that includes everything your insurance requires:
Provider information (NPI, Tax ID)
CPT codes
Diagnosis (ICD-10 codes)
Date and cost of service
Proof of payment
Any and all supplementary documentation needed for approval
We’re also happy to guide you through the submission process if needed.

At PFPT, payment is made directly at each visit using cash, credit/debit card, Venmo, or Zelle. Card payments are processed securely through Stripe (directly or via our Spry system), and you may have a card kept on file for convenience. After payment for a physical therapy session, we provide a superbill for you to submit to your insurance for potential out-of-network reimbursement.

At PFPT, our treatment model focuses on identifying the true “why” behind your symptoms, not just treating where you feel pain. By looking at how your whole body works together, we create a more precise, efficient plan that restores movement, reduces compensation, and improves long-term performance.

PFPT treats orthopedic pain and movement issues from head to toe—including neck, back, shoulder, hip, knee, sports-related injuries, and movement limitations. We focus on identifying the root cause of pain and optimizing how your body moves so you can get back to living, training, and performing at your best.

Wear comfortable clothing that allows you to move easily. Shorts or loose/rollable pants are ideal, especially for lower body issues like knee pain. Please also bring your PT script and any relevant imaging or medical reports to your first visit at PFPT.

No, PFPT goes beyond traditional physical therapy. In addition to rehab, we also offer performance training and strength & conditioning, injury prevention, return-to-sport progressions, wellness and longevity-focused care, movement optimization, and remote/online coaching and programming.

We currently have two locations.

At PFPT, we ask for at least 24 hours’ notice for any cancellations or changes. Late cancellations (under 24 hours) will have a $50 fee, although we understand life happens and can make exceptions for emergencies or unavoidable situations on a case-by-case basis.
